Hence (because the triad is unreal, as in the above verse)
अहं–मुखाःahaṃ–mukhāḥ
beginning with ego
देह–अवसानाःdeha–avasānāḥ
ending with the gross body

and all sense objects
विकाराःvikārāḥ
are the modifications
प्रकृतेःprakṛteḥ

क्षणेkṣaṇe

अन्यथा–भाविनःanyathā–bhāvinaḥ
changing into another form
एषः आत्माeṣaḥ ātmā
This Atman
न उदेतिna udeti
is not born
न अप्येतिna apyeti
does not die
कदा अपि नkadā api na
never

Translation
Hence (as the triad of knower–known–knowledge is unreal) all things starting with ego and ending with the (gross) body and also the sense objects are modifications of prakṛti and they always undergo change. The Atman, on the other hand, is not born, does not dies and never undergoes change.
Annotation
The modifications of prakṛti are called unreal as they constantly undergo change. They appear as real but on examination they are found to be changing and hence unreal. Atman never deviates from its nature (Jagadguru).
